    Label dose schedules

    Zepbound and Mounjaro (tirzepatide)

    2.5 mg once a week for 4 weeks, then up 2.5 mg at a time after at least 4 weeks on each dose: 5, 7.5, 10, 12.5, and a maximum of 15 mg. The 2.5 mg starting dose is for getting used to the medication, not the long-term dose.

    Wegovy (semaglutide)

    0.25 mg for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16, then 2.4 mg from week 17. That's 16 weeks to reach the maintenance dose.

    Ozempic (semaglutide)

    0.25 mg for 4 weeks, then 0.5 mg. If more is needed, 1 mg and then 2 mg, each after at least 4 weeks on the previous dose.

    Compounded tirzepatide and semaglutide

    Compounded plans vary by prescriber. Many follow the same steps as the brand-name label, and some use smaller in-between doses. Enter your own steps above, and add your vial's concentration to see units for each dose.

    Common questions

    Do I have to go up every 4 weeks?

    No. Four weeks is the minimum time on each step, not a requirement to increase. If side effects are rough, or the current dose is working, many prescribers keep you there longer.

    Why does it start so low?

    Starting low and stepping up slowly gives your stomach time to adjust, which reduces nausea and other side effects. The first dose usually isn't expected to do much for weight.

    What if I miss a dose or need to restart?

    Each medication has its own missed-dose rules, and a restart after a longer break may mean going back to a lower dose. Ask your prescriber or pharmacist before restarting.
